Ovarian dysgerminoma is the subject of this case report. Malignant GCTs are divided into subtypes, with dysgerminoma being the most frequent. The lymphatic system is the most prevalent route of dissemination into the paraaortic lymphnode. For six months, a 22-year-old lady presented with amenorrhea and a tumour in her abdomen. A big abdominal tumour measuring 36 weeks was discovered during her health examination. Following that, a computed tomography scan revealed a 222.332.4 cm mixed density lesion in the pelvis-abdominal region, as well as several paraaortic and mesenteric lymph nodes with extensive pleural effusion. Total abdominal hysterectomy, right salpingo-ophorectomy, tumour mass excision, partial omentectomy, and lymphadenectomy were all part of the treatment. Based on histopathological findings, dysgerminoma was suspected. For patients at low risk, postsurgical clinical surveillance can be planned to prevent secondary malignancies, sterility, and gonadal dysfunction caused by pelvic radiation, but diligent followup is necessary to assure early diagnosis of recurrence.
